A. General Informationand Professional Scope
Nutritional Therapy and Health Coaching are complementary approaches that support health and wellbeing through personalised, holistic care. Practitioners use a range of tools to explore lifestyle, nutrition, emotional wellbeing, and health history in order to identify patterns or imbalances that may be contributing to a client’s concerns. The approach is personalised, as practitioners consider each individual to be unique, recommending bespoke nutritional and lifestyle programmes rather than a 'one size fits all' approach.
Nutritional Therapy and Health Coaching recommendations are tailored to the individual and may include recipes, shopping lists, supplement recommendations, dietary models, functional testing, GP referrals, blood tests, and other naturopathic modalities.
Naturopathy is recognised as a complementary medicine. Consultations with a Naturopathic Therapist and Health Coach include taking a detailed case history, recommended treatment plan and following up on results and progression.
Creating a lifestyle guidance, nutritional support, health coaching, or other complementary approaches appropriate to the practitioner’s training and scope.
B. Limitations, Risks, and Referrals
Non-Medical Practice: Nutritional Therapy and Health Coaching is practised by non-medical professionals and is complementary to other regulated forms of healthcare in England.
Diagnosis and Treatment: Nutritional Therapists and Health Coach practitioners do not diagnose medical conditions or claim to treat disease. Any support provided is complementary in nature and focused on wellbeing, lifestyle, and self-care.
Medical Advice: Practitioners’ services/ recommendations are not a substitute for professional medical advice or a replacement for medical advice.
Referrals: Nutritional Therapists and Health Coach practitioners will always refer any client with serious signs or symptoms that fall out of the scope of safe complementary practice (‘red flags’) to their medical professional. Harriet Cooke (trading as Harriet – Nutritionist – Health Coach) reserves the right to determine which cases fall outside their scope of practice, in which event an appropriate referral will be recommended.
Efficacy and Risk: No claim is made as to the efficacy of any Nutritionist and Health Coaching protocols. The degree of benefit obtainable may vary between clients with similar health problems. While the best course of action is continually sought, there always exists the possibility of side effects, adverse reactions, or inefficacy of treatment.
6. Safety: Harriet Cooke (trading as Harriet – Nutritionist – Health Coach) holds your safety and well-being as their top priority in the management of your case.
C. Practitioner Responsibilities
Intervention: Harriet Cooke (trading as Harriet – Nutritionist – Health Coach) will strive to deliver the safest and most effective interventions for your case.
Collaboration: Nutritional Therapists and Health Coach practitioners frequently work alongside medical professionals and will communicate with other healthcare professionals involved in the client’s care to explain any programme that has been provided.
Scope: Harriet Cooke (trading as Harriet – Nutritionist – Health Coach) reserves the right to determine which cases fall outside their scope of practice, in which event an appropriate referral will be recommended. Harriet Cooke (trading as Harriet – Nutritionist – Health Coach) will make it clear if he/ she intends to step outside the boundary of the consultation covered by this document.
Governance: Standards of professional practice in Naturopathy are governed by the General Naturopathic Council (GNC).
Commission Disclosure: Harriet Cooke (trading as Harriet – Nutritionist – Health Coach) may recommend food supplements and/or functional testing as part of your programme and may receive a commission on these products or services. If the practitioner recommends supplements/tests and charges for them, there may be a mark-up on wholesale prices to cover the cost of supply and interpretation.
Insurance Statement: Harriet Cooke (trading as Harriet – Nutritionist – Health Coach) is insured for professional liability and malpractice under policy number ZUR-ANP/26/04/86 with Balens.
.Record Retention Period: Client records are retained securely for 7 years following the last consultation, in line with GNC and GDPR requirements.
Complaints Procedure: If you wish to make a complaint, please contact Harriet Cooke (trading as Harriet – Nutritionist – Health Coach). If unresolved, you may contact my professional association, The Association of Naturopathic Practitioners (ANP) or regulating body the General Naturopathic Council (GNC) for further guidance.
D. Client Responsibilities
The Client understands and agrees
Medical Disclosure: I will inform Harriet Cooke (trading as Harriet – Nutritionist – Health Coach) of all medical conditions I have been diagnosed with, symptoms I am experiencing, and medications I am taking/have taken in the past. I must inform them of any new medical conditions or symptoms or medications should they arise. Providing full details of diagnosed medical conditions and medications is crucial, as this may affect the nutritional programme.
Pregnancy/Breastfeeding: I will inform Harriet Cooke (trading as Harriet – Nutritionist – Health Coach) immediately if I am pregnant or breastfeeding, or if I plan to become pregnant or begin breastfeeding.
Communication with Medical Professionals: I am responsible for contacting my GP about any health concerns. I should tell my GP, or any other medical provider, about any nutritional or lifestyle recommendations provided by my practitioner, as this is necessary because of any possible reaction between medication and the nutritional adjustments. I am directed to promptly consult my doctor or equivalent medical professional if I have not previously done so regarding my condition.
Prescribed Treatments: I am not to alter or discontinue treatments prescribed by a licensed health professional without consulting the individual who prescribed the treatment.
Understanding and Compliance:
I will inform Harriet Cooke (trading as Harriet – Nutritionist – Health Coach) if I
do not understand any given part of the recommendations or if I am uncomfortable with any aspect of my care. The suitability of the advice depends on the accuracy of how I answer questions during the consultation and complete the questionnaire. Responsibility cannot be accepted if important information is withheld or in cases of non-compliance.
I understand that the recommendations are personal to you and may not be appropriate for others.
Recommendations for supplements are entirely my responsibility. I must contact my practitioner should I wish to continue any specified dietary or supplemental programme for longer than the original agreed period, to avoid any potential adverse reactions.
6. Product Purchase: I am free to purchase any products recommended for my treatment from a vendor of my choosing, being under no obligation to purchase products from Harriet Cooke (trading as Harriet – Nutritionist – Health Coach) directly.
E. Fees, Payment, and Cancellation Policy
1. Fee Schedule: I have read and understand Harriet Cooke (trading as Harriet – Nutritionist – Health Coach) fee and prices schedule. To secure bookings the full price of the package will be charged. In the event of a cancellation outside the 48-hour cancellation policy, this will be refunded in full.
A reminder email and text message will be sent 72 hours prior to any consultations that last 45 - 90 minutes. Reminder emails will be sent 24 hours prior to any other scheduled events, such as explanation calls or check-in calls.
2. Consultation Fees:

Initial consultation of 90 minutes
Plan explanation 20-30 minutes
Optional 15-minute check-in call
Follow-up consultation of 45 minutes
Plan explanation 10- 20 minutes (if required)

● Year-Long Support Package - £330
Initial consultation of 90 minutes

Five follow-up consultations of 45 minutes Plan explanation 10- 20 minutes (if required)
Young Persons Discount
Clients aged 21 years or under at the time of booking are eligible for a 10% discount on the total package price (“Young Persons Discount”).
Eligibility for the Young Persons Discount must be confirmed at the time of booking, and proof of age may be requested.
The Young Persons Discount is applied at the sole discretion of Harriet Cooke (trading as Harriet – Nutritionist – Health Coach) and may be withdrawn, amended, or refused at any time without prior notice.
3. Additional Costs:
Additional costs must be set, clear, and transparent to the client. Such fees include any fees for cancellation of an appointment, information regarding any deposits taken, cost of herbal tinctures or supplements or tests sold by you. Biochemical tests and/or nutritional/herbal supplements (if agreed upon) must be paid for at the end of the consultation via a Stripe payment link.
3. Cancellation Policy: You may cancel any appointment but should try to give more than 48 hours notice. If you cancel with less than 48 hours you will be charged £24.
Follow-ups which are included in the packages, are set in price. Any failure to reschedule or book the follow-up included in the package is not subject to refund or reduction in price fornot completing the package.
F. Confidentiality and Data Protection (GDPR)
Confidentiality: All of the information I provide to Harriet Cooke (trading as Harriet – Nutritionist – Health Coach) is protected by General Data Protection Regulations (GDPR) and is confidential unless disclosure is required by law. Harriet Cooke/ Harriet - Nutritionist - Health Coach will keep your personal information confidential and secure following the GDPR guidelines.
Case Reports: My case information may be used for the publication of case reports or case studies. Any information concerning my identity will be excluded from publication, thus maintaining my anonymity.
Disclosure for Harm: Harriet Cooke (trading as Harriet – Nutritionist – Health Coach) will not share your information with third parties without your consent. However, if there is a belief that there is a risk of significant harm to yourself or another person, the information may be passed onto an appropriate authority using the legal basis of vital interest.

I am the data controller for the personal information I collect and process in the course of providing the professional services.

Special categories of data included race, ethnic origin, politics, religion, trade union membership, genetics, biometrics (where used for ID purposes), health and sexual orientation.

Your data may be shared with other parties for the following reasons if you have consented to in the GDPR consent form:For booking and administrative purposes.
To courier and logistics providers who ship products to you.
To companies supplying supplements or other products directly to you on my
recommendation.
To obtain functional tests (such as blood or urine).
6. How Your Information Is Stored
All personal information is stored securely in:
Locked filing cabinets (for paper records) accessible only to me.
7. Online Consultations and Electronic Communication
Online consultations are conducted using reputable platforms with appropriate security measures in place. Although all reasonable steps are taken to protect your information, no internet-based communication system can be guaranteed to be completely secure. If you choose to communicate via email, please be aware that standard email is not fully encrypted.
8. How Long Records Are Kept
In line with professional standards and insurance requirements, clinical records are retained for:
7 years from the date of last consultation
For children: until age 25 (or 26 if aged 17 at end of treatment)
After this period, records are securely deleted or destroyed. In certain cases, such as where records may be relevant to an insurance claim or legal proceeding, they may be retained for longer.
9. Data Regulations for Minors
Where the client is under the age of 18, consent from a parent or legal guardian is required before treatment can begin. The child remains the data subject under data protection law.
Both parents may have the right to access the child’s records unless there is a legal restriction or court order in place that limits this access.
10. Sharing Your Information
I will not share your information with third parties unless:
You have given explicit consent (for example, to share with your GP or another healthcare provider);
Disclosure is required by law (for example, in cases of serious risk of harm);
It is necessary for accounting or administrative purposes (e.g., my professional
indemnity insurer or accountant, who are GDPR-compliant). 11. Your Rights Under UK GDPR, you have the right to:
Access the personal data [I/we] hold about you.
Request to move, copy or transfer your data to a third party.
Request correction of inaccurate information
Request deletion of your data (where legally permissible)

Please note that clinical records cannot be deleted where retention is required by law, insurance, or professional standards.

13. Reporting breaches
Any breach of this policy or of data protection laws will be reported as soon as practically possible. This means as soon as we become aware of a breach.
I have a legal obligation to report any data breaches to UK Supervisory authority which is the Information Commissioners Officer within 72 hours.

If we do not receive this consent, we will not be able to coordinate your care with other practitioners, which may reduce the effectiveness of your healthcare.
I consent to my relevant health information being shared with other healthcare providers involved in my care.

I consent to my relevant health information being shared with my GP, if appropriate.
We may also need to share your contact details with laboratories or biochemical testing companies to order tests as part of your healthcare. Some testing providers may be located outside the UK or European Economic Area (EEA).
